About the Event

Come learn about Unite Us and how we are connecting health and social care providers across our state to improve the health of our neighbors. This session will provide information about Unite Us, offer some open dialog to determine your goals to meet the needs of the community you serve, as well as a quick demonstration of the platform. We will also allow time to network with other attending organizations. We would love to connect with you to learn more about your goals and explore how we can work together.
