Mountain Central Community Information Session

About the Event

Please join us for a Community Information Session to learn more about the coordinated care network rolling out across North, South Dakota, and Wyoming. Unite North Dakota, Unite South Dakota, and Unite Wyoming are coordinated care networks, which allow organizations to send and receive secure electronic referrals, track outcomes, improve community health, and wrap-around social services by connecting individuals to more resources. Joining a network is free of cost for most community-based and nonprofit organizations across the state.
