Join us at an upcoming Community-Based Organization Information Session to learn about CommunityCares, Arizona’s statewide referral system to address social determinants of health. This new technology platform, powered by Unite Us, is designed to connect organizations across sectors, from community service providers to healthcare to government agencies to streamline the referral process, foster easier access to vital services, and provide confirmation when social services are delivered. Non-profit community based organizations participate at no cost and are eligible for a $17,000 Incentive Program from Contexture, Arizona’s health information exchange.
