Alliance for Health Equity & Unite Us Community Information Session

The Alliance for Health Equity and Unite Us have partnered to increase access to critical services in Chester County but we need your help! Join this session to learn more about the initiative, see a demonstration of the platform, and learn how to get involved. Unite Pennsylvania is a coordinated care network of health and social care providers. Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people's social needs, and improve health across communities.
